2025 Winter Holidays

In observance of the winter holidays, ABFAS office will be closed from Wednesday, December 24th through Friday, January 2nd. We will reopen on Monday, January 5, 2026, at 7:00 am Pacific Time.

If you will require the addition of funds to your pre-paid verification account, the request and payment must be received by Friday, December 19, in order to be processed.

To purchase pre-paid verifications, download the Block Verification Pre-Payment Form and submit it along with credit card payment information via email or fax to ABFAS. If paying by check, please mail the form with your payment to ABFAS’ bank lockbox address listed on the form.

All forms and payments must be received by Friday, December 19 in order to be processed.  All forms/payments received after this date will be processed on Monday, January 5, 2026.

Effective January 5, 2026, each verification request per practitioner costs $50.
